OBJECTIVE: To describe a successful urgent endovascular repair of a juxtarenal abdominal aortic aneurysm with contained rupture. DESIGN, SETTING, AND PARTICIPANTS: A hybrid suite using a surgeon-modified fenestrated endovascular graft and advanced 3-dimensional imaging workstation. The possibility of endoluminal treatment of a juxtarenal abdominal aortic aneurysm with partially covered spiral Z stent was assessed in experiment and in three clinical cases. In the experiment, the noncovered spiral Z stent was placed into the abdominal aorta, across the origins of renal arteries and mesenteric arteries, in six dogs.
As the aneurysm gets larger, the risk of it rupturing also increases. Unfortunately, this is how some patients discover the presence of an aneurysm. Most abdominal and thoracic aneurysms are detected on scans performed for other reasons. Abdominal aortic aneurysms (AAAs) can be imaged using abdominal ultrasound scans, CT and MR angiograms.
RESULTS: Findings from serial follow-up CT and laboratory analysis performed in all patients and captopril renography performed in five of the patients who underwent juxtarenal stent-graft placement did not reveal impaired renal function or perfusion. CONCLUSION: Successful exclusion of abdominal aortic aneurysms located closer than 15 mm to the orifices of the renal arteries is possible with juxtarenal placement of the uncovered portion of the stent in the abdominal portion of the aorta. Abdominal aortic aneurysms (AAAs) are classified as juxtarenal if their proximal extent is next to the origin of the renal arteries but does not involve them. An AAA is suprarenal if it extends above at least one renal artery and ends below the celiac axis.
